The H and E-stained image from the renal parenchyma shows a glomerulus with dilated capillary lumina obliterated by fibrin thrombi.
This is the gross cut section image of the unutilized kidney showing flea-bitten appearance corresponding to diffuse glomerular microthrombi.
Glomerular microthrombi in preimplantation biopsy are donor derived -Most common cause- DIC-especially in cases of head trauma and brain death. They are thought to usually resolve by patients own fibrinolytic system. There is no clear acceptance criterion in literature for such organ. Study by Guofeng Gao et al, found 92% recipients of deceased donor allografts with diffuse glomerular fibrin thrombi experienced good clinical outcome..
